Document set creating error

Hello! I need your help) I have configured my Document Set in Sharepoint library. Also, I have configured some SP Forms settings for this Doc Set form. After that, i have Document Set saving error. Error text is "Value cannot be null. Parameter name: strFolderName". What I need to do?

Hello Andrew,
What's your SharePoint edition - online or on-premises? What's your Forms Designer version (you can find it in the lower-right corner of the designer window)? Could you provide a screenshot of the error and URL of the page?

Sharepoint edition on-premises. Version of SP Forms is 3.1.3.

Hi,
Could you try to remove fields one-by-one from your form to figure out which one causes the error?
