Embedding Form into site

We're trying to embed a SPForm into a sharepoint page but getting an error

I think I'm doing everything right - Any Ideas??

This is not a Forms Designer web part, it is a web part for Plumsail Forms, which is supported here - https://community.plumsail.com/c/forms

Are there any errors in browser's console when you select the form for the web part? Do you use Plumsail Forms or Forms Designer? Can you send us a screenshot of the Critical Machine Downtime New form?

If you want to embed a Forms Designer form, this is done via exporting form, and will only work on classic pages. More on that here - https://spform.com/styling/publish-desi ... te-updated
