Wrong edit form content

I have a redirect to the Edit form on Submit button. Recently users complain that sometimes wrong form is opened, not the one just created but some of the old entries. Url is correct and shows the new item id. As it doesn't happen always I'm not able to reproduce this. Any ideas what might cause this and how to solve?

Is it the original form and list? Or is it created from the template?

Also, please ask users to completely clear browser cach.
