Hello Roo,
Most probably, the issue is related to the SharePoint list itself.
Have you created it from the template?
I would suggest creating a new list from scratch using the same field names and import existing forms to it.

Why are you blaming sharepoint if a SPForm is not working? it seems an easy way out?
I don't really want to be recreating libraries, powerflows and shortcuts everytime a SPform doesn't work.
Wouldn't it be better to try and solve this?

If the problem occurs in only one SharePoint list within the site, the root cause might be related to this specific SharePoint list.
That is why I'm asking have you created it from the template or programmatically? Or have you created this list from scratch?
Have you tried to restore the default SharePoint forms for this list, not forms designed with Forms Designer?
