Hi,

I am having difficultly linking to a Forms Designer Form within a SharePoint Designer Workflow Email Action.

Can you please tell me how to do this?

Not sure that I understood you. You cannot link to a form from a workflow. You can link to list items from a workflow. Say start a workflow when an item has been created or updated and retrieve its properties.
