New form not updating

Hello - having an odd issue where the list stops using my custom spform and only uses the default list form. Can edit and save the spform but the list doesn't use it. Details:

- content type: Item

- form: New

- group: none, all users

- item content type is associated with the fd form controllers

The custom form worked fine initially but at some point the list stopped calling it.- now only displays the built in NewForm. Could just recreate the list but this has happened a couple of times now so would like to identify a cause/quick solution.

Hi,

Please, check forms' URLs in the list settings and the content type settings in SharePoint Designer.
