Cannot add a form to a page

I have created a site with two lists. I have connected a SP Form to each list. The first form I have added to one page - without any problems.
Next, I have created a new page on the same site and tried to insert the second form. Unfortunately I get the following message: "No item exists at ... It may have been deleted or renamed by another user"
I have tried to:
* Restart my browser
* Tried from different computer
* Rebuild the form
* Used another name when I export the form*
* Created a number of new pages
None of above has helped me.

Thanks for the video. Could you try to create a page without paces in its URL?

Is it possible to provide temporary access to the site with the site collection admin permissions? If so, please, send the credentials to support@spform.com.
