Attachement column not appearing after saving form

Above is a pic of how the form looks like in Forms Designer below is how the form looks like in SharePoint , I have enabled that option that allow users to be able to attach the files. What could be the issue please assist

Use 'Attach' button in the ribbon to add an attachment.
