Unable to upload Attachments to list items

Hi,
we have the follwing issue with SPFORMS in O365

1. starting create new list item
2. click on "Attach file"
3. pick the file for upload
4. click OK and nothing happens
following mesage is being shown in the Console

Did you add Attachments field to the Form?
