I'm wanting to create a form with a button so that when I click it, "Completed" will show up either next to or below it. I want to have different buttons with different department names on them so when the corresponding department has completed when they need to do, they click their button, "Completed" shows up next to it, and I can keep going back to the item and keep track of which departs have completed what they need to complete. Is this possible?

Sure, you can do it with Forms Designer. Just create a new field 'Completed by', put it into your form with Forms Designer and hide with CSS. Place button for each department and get 'Completed by' field filled by JavaScript when user clicks an appropriate button. You can also add 'Completed' text below the button with JavaScript.
