Unable to split the field control

Hi Team,
I am trying to insert the existing list column/field into forms designer like one control for field name and other control for input box. Please refer the below image.. it should be like second field. Any help would be highly appreciated. thank you in advance.

First, select the Field and set its Title to Hidden:

Next, drop a Table control onto the Form, place your field inside the Table and add Plain/Rich Text Control next to it. If it already was inside a Table, you can drop new Table inside existing Table, that's not a problem. Adjust the width of New Title to be closer to your field, either manually or by selecting Auto:
