It seems that you have multiple groups of forms. Please, provide their rules.

I have four groups. All users, Designers, Coordinator and Managers. These are based on SharePoint groups.

Have you customized the list view and the New item link in particular? Could you provide markup of the link (onclick and href attributes)?

I haven't modified anything. How do I get this information for you. I opened the view in SP Designer and can't find the +new item image or link..

Open HTML-source of the view and find the link there.

Please, follow the steps:
Remove all form files beginning with 'fd_' via SharePoint Designer.
Re-save the forms in Forms Designer
If it does not help, pay for the support minutes:

And provide temporary access to the site with the site collection admin permissions.
