1) Can you please tell us how exactly you open the form? It shouldn't happen if you just click New Item or Edit an item, so it's strange to see such behavior.
2) If you add listItem.update(); before renaming the file, the fields should update, though you still wouldn't see it until you close the form. Do not remove second listItem.update(); after renaming the file though.
3) This doesn't work with dialog mode for the New Form due to how dialog forms function. When you click Save, if there are no errors on the form, dialog window just closes and no additional functions can be used after it. The only way it will work for dialog mode is if you use Edit Form.
