Dear Alex,
I see that your URL simply doesn't contain RootFolder parameter, only in Source, meaning only of the place you were redirected from to this form. As such, it's not possible to get current location from URL.

That's rather unfortunate, but it's still possible to do with JSOM, check out this example here on how to get file location in SharePoint - https://sharepoint.stackexchange.com/qu ... stitem-one

If you are able to get the path to the file, including the folder, you should be able to display PDF on the form. Implementing this solution will take us a significant time though, so if you want us to write a code for you, please, leave a request at support@spform.com so we can evaluate the time it will take.
