Attachments stopped working with form

I have a weird issue: the attachment stopped working if i have a form. I tested numerous times, even created a brand new site... Standard list is functioning correctly - attachments are added. As soon as i add a form (no scripts or CSS design) the attachments just stop: clicking ok button doesn't do anything and they are not attached. Do you know what it might be?

Can't be certain, but make sure that you add Attachments field to the Form first:

It is added, and feature is active on the list - as i am saying it works on the list without form, but as soon as i add the form (very basic, no scripts) attachments are not attaching.

Sorry.... i don't know what is going on: it wasn't working for three days - i was trying to troubleshoot and test it every way i can find.... I reinstalled the Form designer app (had a coffee ) and it is working now! Sorry for bothering....
