How can I send form details to an email address upon submission?

I’d like the form to flow into an email using the format laid out below:
Email (Fields in Yellow should be static):
From: [capture field from SUBMITER NAME]
CC: [capture field from EMAIL ADDRESSES OF INDVIDUALS YOU WOULD LIKE CC'D ON OUR RESPONSE]
To: LICHP.0000.NA.US.EN.MSF.RMD.CS.T02.SPT.00.EM@css.one.microsoft.com
Subject: [New Form] + [capture TITLE field from intake form]
Attachments: [capture ATTACHMENTS RELATED TO INQUIRY]
Body: The output of the (HTML?) table created in the link you provided

Hello, Shariq!
This isn't really related to our product as currently Forms Designer offers no additional functionality for MS Flow, but you can use default triggers and actions just like you could for the default form.
You'll need to use one of the following triggers to trigger Flow when an item is created or modified: And then use an Outlook action to send an email with information from the newly created Item.
