Child 'Related Items' are not being deleted

On one of our applications, related child items are not being deleted when the parent is deleted from within the form. The delete msgbox is also different, more like the default delete message than the one Form Designer usually shows when the parent is deleted.

There are two related item tabs, and the related items settings are generally set to default values. One of the child entries does have a cross site lookup field.

Dear Daniel,
Related Items children are tied to parent with Lookup usually, but they are separate items, so they are not deleted by default when the parent is deleted. If you want them to be deleted, you'll need to go to Children list, go to List Settings, select Lookup column used to tie children to parent, select Enforce relationship behavior and pick Cascade Delete:
