Please download and install the latest version of Cross Site Lookup. If you're using SharePoint On-Premises, then you'll also need to reactivate the Cross Site Lookup feature (Go to Site Settings -> Site collection features -> Cross Site Lookup (Click Deactivate and then Activate)).

Please, try to deploy the solution from WSP-package:
http://spform.com/media/default/install ... Lookup.wsp
by following the instructions:
http://spform.com/documentation/sp2013installwsp
Next, re-activate Cross-site Lookup feature at the site collection level.

Hi,
Please, ensure that you're using the latest versions of both Forms Designer (3.0.1) and Cross-site Lookup (1.4.0). Try to re-activate Forms Designer feature at the site collection level and re-save the form. If it does not help, let's schedule a screen sharing session. Drop the connection settings to support@spform.com.
